After we receive your forms we will send you Form F "Statement of Funeral Goods and Services Selected" you may pay via one of these credit cards: Visa, MasterCard, American Express, or Discover, Our office will contact you during regular business hours.

Receive cremated remains as per your request on Cremation Authorization forms.
